Integrated Services Network

The Integrated Services Network (ISN) Support Services Brokerage is a community organization formed by two nonprofit organizations: Catholic Community Services and Chehalem Youth and Family Services. ISN members have pooled resources into joint service contracts in order to improve the quality and cost effectiveness of the service delivery. The ISN Support Services Brokerage is a leader in the transformation of health and human services for adults with Intellectual and Developmental Disabilities (IDD) and believes in self-determination, the ability of people to determine and direct their own lives. ISN staff work collaboratively with customers to identify and access resources to address their support needs and person-centered goals.

    Oregon Department of Human Services

    The Department of Human Services (DHS) is Oregon’s principal agency for helping Oregonians achieve wellbeing and independence through opportunities that protect, empower, respect choice and preserve dignity, especially for those who are least able to help themselves.     Resource Connections of Oregon

    Support Service Brokerages are responsible for linking customers with qualified providers to help them achieve their goals. Wherever possible, the customer will use natural supports already available within the community. However, when necessary, the customer may use funds to purchase these supports from a qualified provider. The customer decides from whom (s)he would like to receive service. The provider of the service must meet a strict criteria of qualifications in order to be paid with government funds to perform the supports.

    RISE

    RISE provides services for people with disabilities including residential settings, day programs, employment assistance, managed care, and home and community based services. RISE also provides services for children and families through foster care and professional parenting, adoption, kinship care, after school and summer programs, behavior supports, and mental health services. In addition, RISE provides Early Intervention services which includes Occupational Therapy, Speech Therapy, Physical Therapy and Developmental Specialists.
